Laparoscopic prostatectomy: transperitoneal approach: for which purpose?

Since Schuessler et al. reported the transperitoneal laparoscopic radical prostatectomy, this procedure is now considered the standard of care for the treatment of localized prostate cancer in many centers. Erdogru et al. suggested that laparoscopic radical prostatectomy causes less postoperative pain, a shorter catheterization time and hospital stay as compared to conventional radical prostatectomy. Nowadays, the laparoscopic procedure is considered feasible and reproducible with long-term functional and oncologic outcomes that appear to be equivalent to open surgery. Transperitoneal laparoscopic radical prostatectomy has its advantages: it allows for a faster placement of trocars and bladder mobilization, which is convenient for a tension-free anastomosis; the larger cavity allows placement of the specimen bag out of the operative field, improving vision and facilitating the vesicourethral anastomosis and extended lymph node dissection. Debate over the merits of transperitoneal vs. extraperitoneal approaches to laparoscopic prostatectomy continues, but the laparoscopic transperitoneal approach, although technically challenging, has proven to be an excellent alternative for dedicated centers wishing to provide a minimally invasive surgical option to their patients with localized prostate cancer.
