The urachal cyst, when persistent and communicating with the bladder, may occur late in a child’s life. When occurring, it is accompanied by abdominal pain, hematuria, and signs of urinary infection.

2. Operative setting 00'33''

Laparoscopy is undertaken with the child placed in a frog-like, supine position at the table’s end. The monitor is placed at the child’s feet and the surgeon stands to the child’s left. Two operating 5mm ports are placed on each side of the umbilicus. A 5mm scope is placed through the umbilicus.

3. Urachal cyst excision and peritonization 00'54''

This is the case of a boy aged 5 and a half. The urachal cyst keeps a persistent, yet very narrow communication with the bladder, as evidenced by the ultrasonography. The urinary endoscopy helps to identify a bulging dome on the anterior surface of the bladder, corresponding to the pressure exerted by the urachal cyst. The communication with the bladder is demonstrated by the presence of an orifice as small as a pinhead. The subperitoneal dissection of the urachal cyst will allow to grasp its loop at the junction between the bladder and the urachus. The procedure ends with the peritonization of the dissection area before the specimen is extracted through the umbilicus after cyst puncture.
