In this key lecture, Professor Bernard George introduces the rationale behind the lateral approach of the craniospinal junction. He presents the pearls of the complex anatomy of the craniospinal junction and vertebral artery. He describes the stepwise technique of the posterolateral and anterolateral approach to the foramen magnum as well as their respective indications.
