Prostate cancer cT1c: what about the lymph nodes?

Today, pelvic lymph node dissection (PLND) is associated with a limited value and no clinical consequences, but there are clear indications for LND (Gleason score higher than 7 and PSA > 10 ng/mL). When the decision of PLND is made, an extended PLND has to be considered standard, the limited PLND is obsolete. The sentinel lymph node dissection is a new alternative for extended LND, that allows the identification of micro-metastases with lower associated morbidity.
