Use of a new grasper in laparoscopic gynaecologic procedures

1. Introduction 00'12''

In this video, we show the use of a new grasper with 3 functions in gynecologic laparoscopic procedures. In fact, instruments are crucial for the progress of laparoscopy. However, changing instruments during laparoscopic procedures leads to loss of time. Multifunction instruments would resolve this issue. We use the new grasper, Robby-type, Karl Storz, Germany in uterosacral endometriotic nodule excision and cystectomy for endometrioma.

2. Endometriotic nodule excision 01'13''

In endometriotic nodule excision, the grasper helps to dissect the pelvic spaces, grasp the nodule for traction, and control hemostasis through bipolar coagulation. All these features can be used with only one instrument with no need to switch between instruments. This leads to time-saving procedures.

3. Endometrioma excision 03'13''

In endometrioma excision, the grasper allows to perform cyst stripping by providing a good grasping and traction forces to keep good hemostasis maintaining clear vision of the operative field by the bipolar coagulation, and to have good dissection of anatomic planes. A study comparing the use of classical bipolar grasper, tooth grasper and scissors to the use of the new bipolar grasper in ovarian cystectomies is ongoing. Primary results show a reduced operative time and a reduced blood loss. Re-usable multifunction instruments would also reduce surgical procedures’ cost.
