Although infrequent, major complications may occur during first trocar insertion during laparoscopy. Immediate recognition and management is critical and may lead to a safe outcome. The objective of this video is to show a severe vascular complication that occurred during laparoscopic cholecystectomy performed by a novice surgeon.
