A morbidly obese patient (BMI of 39) presented with a second recurrence of umbilical hernia, previously treated by simple suture once, and by laparoscopy a second time. A new laparoscopic procedure was performed. It showed the lack of adhesions on the mesh and tackers, and the mechanism of recurrence. A new large double-layered mesh was used to treat this recurrent ventral hernia. The postoperative course was uneventful.
