The objectives of this video are to demonstrate the use of probe-based confocal laser endomicroscopy (pCLE) for the intraoperative detection of the distal margin of a middle rectal tumor and to show the steps of a pure transanal TME in a 71-year-old woman, who presented with a T2N0 middle rectal adenocarcinoma. Confocal imaging is performed prior to surgery, and clips are placed on the microscopically proven healthy mucosa.
