This video shows the laparoscopic repair of a giant type III paraesophageal hernia (PEH). The 76-year-old patient underwent a chest CT-scan for dyspnea which revealed a type III PEH with mesenteroaxial volvulus of the stomach. The patient had no symptoms of gastroesophageal reflux disease (GERD) and esophageal function tests were normal. Dissection of the hernia sac was performed using a central approach and a fundophrenicopexy was performed since there was no evidence of GERD.
