We present a laparoscopic Nissen fundoplication for gastroesophageal reflux disease (GERD) in a patient with a BMI of 21 with a very nice demonstration of the relevant anatomical structures. Numerous important tips and tricks to prevent complications or failure of a fundoplication are given during the various steps of the procedure. The intraoperative endoscopic controls show hyperplastic polys due to chronic proton pump inhibitor (PPI) treatment, serve to localize the Z-line, and to confirm the optimally positioned fundoplication.
